Welcome to Youth, operated by Kendall Wellness Center, LLC d.b.a. Youth ("Youth," "we," "us," or "our"). These Terms of Use ("Terms") govern your access to and use of www.findyouth.co, our online booking and patient platforms, our SMS program, and any related services we provide (collectively, the "Services").
Please read these Terms carefully. They include important provisions that affect your legal rights, including a binding arbitration agreement, a class-action waiver, and limitations on our liability. By using the Services, you agree to these Terms.
If you do not agree, do not use the Services.
By accessing the Services, creating an account, booking a consultation, opting into our SMS program, or purchasing any product or service, you accept and agree to these Terms and our Privacy Policy.

The Services are not intended for minors, individuals outside the United States, or individuals located in jurisdictions where any portion of the Services is unlawful.
The Services available to you depend on the state in which you reside. Telehealth, the practice of medicine, and the prescribing and dispensing of medications are each regulated at the state level. The clinicians associated with Youth — including Samantha Alonso, APRN — are licensed in the State of Florida only.
By engaging the Services, you acknowledge and agree that:
If at any time we determine that we cannot lawfully or appropriately serve you, we may decline, suspend, or terminate Services and issue any refund consistent with Section 7.
You are responsible for keeping your login credentials confidential and for all activity that occurs under your account. Notify us immediately at [email protected] of any unauthorized use.
Youth is a wellness and optimization service that combines provider-led telehealth consultations with personalized protocols, including peptide therapy, GLP-1 programs, lab work, and ongoing clinical support. Care is delivered through a concierge-style membership model.
Clinical services are provided by Samantha Alonso, APRN and other licensed clinicians we engage from time to time. Administrative and operational support is provided by the Youth team.
Our public website and marketing materials are operated by Kendall Wellness Center, LLC. Our patient electronic health record system is provided by Practice Better, a HIPAA-covered EHR platform. Once you become a patient, all clinical communications, intake forms, lab results, prescriptions, and protected health information are managed inside Practice Better.
Information presented on www.findyouth.co, in our quizzes, social media, marketing emails, SMS, advertisements, downloadable guides, and any other public-facing material is provided for general educational and informational purposes only. It is not medical advice, does not constitute a diagnosis or treatment plan, and does not establish a provider-patient relationship.
A provider-patient relationship is established only after all of the following:
Until each of those steps is complete, nothing on our website, in our marketing, or in any conversation with our team should be interpreted as medical advice or a recommendation to use any specific product, dose, or therapy.
If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. The Services are not designed for emergency or urgent care.
This Section is one of the most important in these Terms. The Services involve two distinct service models depending on your state of residence, with different product channels, different regulatory frameworks, and different risk profiles. Before engaging the Services, read this entire Section.
You agree not to misrepresent your state of residence in order to obtain a product or Service that is not available to you under your state's law.
When a clinician affiliated with Youth writes a prescription for a Florida patient, that prescription may be fulfilled by:
Compounded medications are not FDA-approved finished drug products. They are prepared by licensed pharmacies under federal and state compounding rules and do not undergo the FDA's pre-market review and approval process that applies to commercially manufactured branded drugs. Pharmacy partners may change from time to time, and the dispensing pharmacy will be identified on the label of any product shipped to you.
For customers outside Florida, peptide and related products are made available through independent third-party suppliers, not through prescription-pharmacy channels. By electing to obtain a product through this channel, you acknowledge:
The U.S. Food and Drug Administration's regulatory positions on peptide compounding, the use of bulk peptide substances, the compounding of GLP-1 receptor agonists, and the distribution of non-FDA-approved peptide products are evolving and have been subject to changing guidance and enforcement priorities. Pharmacy boards in individual states may impose additional or different restrictions. As a result:
Youth is not responsible for changes in product availability driven by federal or state regulatory action.
For Florida patients receiving prescribed therapy, some medications and peptides may be prescribed for indications, doses, durations, or routes of administration that differ from those approved by the FDA for any commercially available product. Off-label prescribing is lawful when based on a clinician's medical judgment but may carry risks that have not been fully characterized in published research. Off-label use will be discussed with you and reflected in your informed-consent forms inside Practice Better before any prescribed therapy is initiated.
For the clinical track, no medication is dispensed unless a licensed clinician evaluates you, determines that the therapy is clinically appropriate, and writes a prescription consistent with applicable law. Marketing materials, quiz results, protocol descriptions, educational content, ad creative, and pricing pages are not prescriptions and do not entitle you to receive any specific product. The individualized clinical determination — not your selection on a quiz or website — is the basis on which any prescribed product is provided.

This acknowledgment supplements, and does not replace, any product-specific informed-consent form you sign inside Practice Better or any acknowledgment a third-party supplier requires at point of sale. If you are not willing to accept the disclosures in this Section 4, do not purchase a peptide, compounded medication, or related product through or with the assistance of Youth.
You agree to:
Discontinuation and medication management. Decisions to start, continue, taper, or discontinue any medication should be made in consultation with your clinician. Once a medication has been dispensed to you, the management, storage, administration, and discontinuation of that medication is your responsibility, undertaken in consultation with your clinician. Youth is not responsible for outcomes resulting from a patient's unilateral decision to discontinue, alter the dose of, or otherwise deviate from a prescribed protocol without clinical guidance.
Youth offers three membership tiers:
Memberships are billed in advance on a recurring monthly basis. Membership pricing and benefits may change; we will provide notice of material changes.
Persistent chat access means you can send messages through Practice Better at any time, and we reply within 24 hours on business days. We post out-of-office notices when reply windows will be longer. "Persistent access" does not mean 24/7 availability or guaranteed instant reply.
Membership versus protocol. Memberships are the care container — access plus check-ins. Protocols (peptides, GLP-1 programs, labs, and other therapies) are billed separately on Practice Better invoices. Membership does not include the cost of any protocol, medication, lab, or pharmacy fulfillment.
Membership cancellation. You may cancel your membership at any time, provided you are not on an active prescribed protocol. Cancellation requests must be sent in writing by email to [email protected] or by SMS to (786) 607-2686. Cancellation takes effect at the end of your current paid month; the current month is non-refundable. If you are on an active protocol, please contact us to coordinate clinically appropriate next steps before cancellation.
Reactivation. Members who cancel and later wish to return are charged a flat $100 reactivation fee in addition to the first month's membership fee.
All bookings, intake, and clinical billing are handled through Practice Better and our payment processor. By booking or purchasing, you authorize charges to the payment method you provide for the applicable fees, including recurring membership charges and any prescribed protocol or service.
You are responsible for ensuring your payment method remains valid. We may suspend Services for accounts with failed or past-due payments.
A $25 no-show / late cancellation fee applies if you fail to attend a scheduled appointment or cancel or reschedule with less than 24 hours' notice.

Voicemail messages and direct messages on social media platforms do not constitute valid notice.
Filing a chargeback for a charge that is consistent with these Terms may result in suspension or termination of Services and collection of any unpaid balance. We reserve all legal remedies for chargebacks initiated in bad faith.
Promotional pricing, introductory offers, and discount codes are subject to their own posted terms and may be modified or discontinued at any time. Promotions are not retroactive.
By opting into our SMS program (by submitting a form, checking an opt-in box, or texting us at the number below), you expressly consent to receive recurring automated marketing, transactional, and informational text messages from Kendall Wellness Center, LLC d.b.a. Youth at our dedicated SMS number (786) 607-2686, including messages sent using an automated telephone dialing system or pre-recorded voice.
Consent is not a condition of purchase or of receiving Services.
Messages may include appointment reminders, intake form requests, payment reminders, refill prompts, clinical-logistics nudges (e.g., "Samantha replied in your Practice Better inbox"), program updates, and occasional marketing.
We do not send protected health information (PHI) — labs, diagnoses, dosing details, or symptom-level conversation — by SMS. PHI lives in Practice Better.
We will not sell, rent, or share your mobile number or SMS opt-in data with third parties for their own marketing purposes. Mobile information is shared only with our service providers (e.g., our SMS platform, our EHR) for the purpose of providing the Services to you.
We may change our SMS short code, long code, or messaging cadence at any time. Continued participation in the program after notice of a change constitutes acceptance.
You agree not to:
We may suspend or terminate Services for any violation of this section.
All content on the Services — including the Youth name and logo, the slogan "Experience Health, Embrace Youth.", website copy, graphics, photography, videos, downloadable guides, course materials, quiz design, protocols, and software — is the property of Kendall Wellness Center, LLC or its licensors and is protected by U.S. and international intellectual property laws.
We grant you a limited, non-exclusive, non-transferable, revocable license to access and use the Services for your personal, non-commercial use. You may not copy, reproduce, modify, distribute, publicly display, or create derivative works from any of our content without our prior written permission.
User-generated content (testimonials, reviews, photos) you submit to us grants Youth a non-exclusive, royalty-free, worldwide license to use, display, and reproduce that content in connection with the Services and our marketing, subject to our Privacy Policy. We will not publish a named testimonial or "before/after" image without your specific consent.
The Services rely on, integrate with, and may link to third-party platforms and tools, including:
We do not control these third parties and are not responsible for their content, terms, availability, or data practices. Your use of any third-party platform is subject to that platform's own terms and privacy policy.
Our collection and use of personal information is described in our Privacy Policy, which is incorporated into these Terms by reference. Once you become a patient, your protected health information is handled inside our HIPAA-covered EHR (Practice Better) under the Notice of Privacy Practices provided to you at intake.
THE SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE." TO THE FULLEST EXTENT PERMITTED BY LAW, WE DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, AND ANY IMPLIED WARRANTIES ARISING FROM COURSE OF DEALING OR USAGE OF TRADE.
We do not warrant that the Services will be uninterrupted, error-free, or free from harmful components, or that any specific therapy, protocol, or product will produce any particular outcome. Individual results vary.
This section does not limit any non-waivable warranty or right under applicable law, including consumer-protection statutes that apply to you.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, in no event will Kendall Wellness Center, LLC, its members, managers, officers, employees, contractors, clinicians (in their corporate capacity, not their individual clinical capacity), affiliates, or service providers be liable to you for any indirect, incidental, special, consequential, exemplary, or punitive damages — including lost profits, lost data, loss of goodwill, or any damages arising from:
Our total aggregate liability arising out of or relating to the Services or these Terms will not exceed the greater of (a) the amount you paid Youth in the twelve (12) months immediately preceding the event giving rise to the claim, or (b) one hundred U.S. dollars ($100.00).
Nothing in this Section limits liability that cannot be limited under applicable law, including, where applicable, professional liability of an individual clinician acting in his or her clinical capacity. Some jurisdictions do not allow the exclusion or limitation of certain damages; if those laws apply to you, parts of this section may not apply.
You agree to defend, indemnify, and hold harmless Kendall Wellness Center, LLC, its members, managers, officers, employees, clinicians (in their corporate capacity), affiliates, and service providers from and against any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising from or related to: (i) your use of the Services; (ii) your violation of these Terms; (iii) your violation of any law or third-party right; (iv) your provision of inaccurate or incomplete information; or (v) your decision to discontinue, alter, or deviate from a prescribed protocol without clinical guidance.
PLEASE READ THIS SECTION CAREFULLY. IT AFFECTS YOUR LEGAL RIGHTS, INCLUDING YOUR RIGHT TO FILE A LAWSUIT IN COURT AND YOUR RIGHT TO PARTICIPATE IN A CLASS ACTION.
Before initiating any formal dispute, you and Youth agree to attempt to resolve the dispute informally for at least sixty (60) days. To start, send a written notice describing the dispute and the relief you are seeking to: Kendall Wellness Center, LLC, attn: Legal, [email protected].
If the dispute is not resolved within 60 days, any dispute, claim, or controversy arising out of or relating to these Terms or the Services will be resolved by final and binding individual arbitration, administered by the American Arbitration Association ("AAA") under its Consumer Arbitration Rules then in effect.
The Federal Arbitration Act governs the interpretation and enforcement of this Section.
You and Youth each agree to bring claims against the other only in your or its individual capacity, and not as a plaintiff or class member in any purported class, collective, consolidated, or representative action. The arbitrator may not consolidate more than one person's claims, and may not preside over any form of representative or class proceeding.
You may opt out of this Section 16 by sending written notice to [email protected] within 30 days of first accepting these Terms, with subject line "Arbitration Opt-Out" and your full legal name and the email address associated with your account. If you opt out, neither party is bound by this Section 16 with respect to you, and disputes will proceed in the courts identified in Section 17.
Notwithstanding the above, either party may: (i) bring an individual action in small-claims court for any claim that qualifies; (ii) seek injunctive or equitable relief in a court of competent jurisdiction to protect intellectual property rights or stop unauthorized use of the Services; or (iii) pursue any claim that, by law, cannot be subject to mandatory arbitration.
These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-laws principles. Subject to Section 16, any action that may be brought in court will be filed exclusively in the state or federal courts located in Miami-Dade County, Florida, and the parties consent to personal jurisdiction in those courts.
We may suspend or terminate your access to the Services at any time, with or without notice, for any reason, including violation of these Terms, fraud, abuse, non-payment, or where we determine we cannot lawfully or appropriately serve you. You may terminate by canceling your account and discontinuing use of the Services, subject to the membership and protocol provisions above.
Upon termination, the rights and licenses granted to you cease, and any provisions of these Terms that by their nature should survive (including Sections 4, 5, 7.3, 10, 12–17, and 19) will survive.
We may update these Terms from time to time. When we do, we will update the "Last Updated" date and, for material changes, provide additional notice (such as email, SMS, or a notice on the Services). Your continued use of the Services after the effective date of the updated Terms constitutes your acceptance of the updated Terms. If you do not agree, stop using the Services and contact us to close your account.
